This is a vintage 90s game shirt designed by illustrator Susumu Matsushita, featuring Captain SSK, the mascot of sports equipment manufacturer SSK.

It features unique American comic-style illustrations using an airbrush, a signature of Susumu Matsushita, an illustrator with fans worldwide who has also designed cover illustrations for the game magazine Famitsu, the Adventure Island series starring Master Takahashi, the mascot for the Nagano Olympics bid, and the mascots for Orix Buffaloes and Gamba Osaka (Gamba Boy).
